Client Consent & Signature Agreement

By requesting services from Route by Riley Logistics (“Company”), I acknowledge and agree to the following:

1. Consent to Transport
   I authorize Route by Riley Logistics to collect, transport, and deliver any approved medical items, documents, or specimens on my behalf. I confirm that all materials sent for delivery comply with federal, state, and local regulations.

2. HIPAA Compliance
   I understand that Route by Riley Logistics is committed to protecting sensitive health information in accordance with the Health Insurance Portability and Accountability Act (HIPAA). I consent to the handling of any personal health information (PHI) as required for delivery purposes.

3. Communication Authorization
   I consent to receive communication by phone, email, or SMS for appointment confirmations, delivery updates, or service-related information.

4. Service Liability Acknowledgment
   I understand that Route by Riley Logistics is not responsible for improperly packaged items, inaccurate pickup/drop-off information, or delays caused by incorrect or incomplete addresses.

5. Digital Signature Validity
   I understand that signing this agreement digitally is legally binding, and that my electronic signature represents my consent.

6. Privacy and Security
   I confirm that I have reviewed the Company’s Terms & Conditions and Privacy Policy, and agree to the collection, use, and storage of my information as outlined.
